Greater Houston Healthconnect Significantly Expands By Adding Memorial Hermann
HOUSTON (PRWEB) April 13, 2018 -- Memorial Hermann Health System has joined the Greater Houston Healthconnect Network, the region’s community-based health information exchange (HIE), in a move that significantly expands the coordination and exchange of health information for patients across the region.
Healthconnect gives Memorial Hermann and other care providers in the community real-time access to historical medical information for patients across the care continuum. As one of the largest not-for-profit health systems in Texas, Memorial Hermann has a robust footprint in the community with 5,500 affiliated physicians and more than 2 million patient encounters annually.
Partnering with Healthconnect extends Memorial Hermann’s ability to coordinate the care of its patients by seamlessly integrating the electronic health record (EHR) systems of other hospitals, health systems, community clinics, physician office practices and service providers. This connectivity ensures that all providers that are part of Healthconnect have the full composite of a patient’s medical information available to them, right at the point of care. Ultimately, this kind of interoperability—across different providers, EHR systems and diverse venues of care—allows clinicians to work together to improve quality, efficiency and patient safety.

Memorial Hermann joins an already extensive portfolio of providers within the Healthconnect network. Patients who consent to sharing their information on the health information network can make that information readily available to participating medical providers when and where it’s needed.
