Caradigm and Rush Health Announce New Health Information Exchange (HIE) for Rush Health Members Aims to Improve Quality of Care and Population Health
Chicago, Illinois (PRWEB) January 14, 2014 -- Rush Health, a clinically integrated network of physicians and hospitals, is creating a private Health Information Exchange (HIE) for its member physicians and hospitals, using advanced population health software to improve care quality, care coordination and patient outcomes throughout its network, the organization announced today. To support this initiative, Rush Health has contracted with Caradigm (Bellevue, WA) for a variety of healthcare analytics and population health software.
“This is not about simply moving health information data from point A to point B,” said Brent J. Estes, president and CEO, Rush Health. “We will be able to identify populations of patients with chronic diseases like hypertension or diabetes, and know right away when patients are seen in an emergency room or admitted to a hospital. Using this information, we can better analyze trends and patterns to organize approaches to care and follow up that can reduce episodes of care, improve health and save patients and payers money.”
Using Caradigm’s software solutions, the Rush Health HIE will give Rush Health physician practices and hospitals access to near real-time clinical, operational and financial data aggregated from multiple technology systems used by Rush Health physicians and hospitals, as well as other providers in the market, including distinct Electronic Medical Record (EMR) systems and Rush Health’s Enterprise Data Warehouse (EDW).
Access to this, near real-time data will enable clinicians to make more informed decisions about the care of individual patients and to improve the coordination of care for patients with complex and chronic conditions as they transition from hospital to clinic to home.
The near real-time data asset also will serve as the foundation for a wide range of advanced analytics and reporting applications that support Rush Health’s quality, performance, accountable care and population health initiatives. For example, Rush Health will expand its provision of quality and performance measures to its 900 member physicians to enable community-wide quality reporting. Other applications will be used to guide risk-based contracting, accountable care operations, care management, utilization management, and the dissemination of evidence-based best practices.
Rush Health’s new Health Information Exchange will be used to share data with external organizations, including payers, National Registries and the MetroChicago Health Information Exchange.
