Anyone can sign up, but to be a certified Transitional Care Manager one has to be a licensed nurse (LVN, LPN, RN). Once training is complete, the graduate will be given instructions on how to get listed as a transitional care manager on the online transitional care service platform with mobile application. The transitional care manager can then go to hospitals and by utilizing the three-step process can provide a smooth transition from hospital to home.
"In step two they will go to the home to do a home safety aid evaluation and make sure the home is ready for the client," concluded Weatherman. "Transitional care managers also educate clients on their self-directed care and go over all follow-up instructions."
