eTransX Involved with New Rural Value-Based Healthcare Initiative in Tennessee
Nashville, TN (PRWEB) April 07, 2014 -- eTransX, a leader in healthcare connectivity and collaboration solutions, today announced its involvement in the development of a new, innovative value-based healthcare delivery model for a coalition of rural hospitals and clinics in upper middle Tennessee. Known as the Upper Middle Tennessee Rural Health Cooperative (UMTRH) project, this initiative is focused on implementing a new collaborative regional system of care that will improve health outcomes, improve the quality of care, and reduce healthcare costs for high cost high-risk Medicare beneficiaries in a six county rural region. The six Tennessee counties are Macon, Trousdale, Smith, Clay, Jackson, and Sumner.
“eTransX has been instrumental in providing key design ideas and strategic assistance in the design of this clinically integrated network model that incorporates the planned use of advanced information technology such as eTXChange (Health Information Exchange solution) and local community care coordinators,” said Bill Mize, the Director of the rural value-based healthcare initiative.
The project mission is to develop a new sustainable care delivery model that will improve the coordination of care for high risk, high cost patients in the coverage area, including Medicare and Medicaid (Tenncare) beneficiaries. This new care delivery network model will address six key components:
1) Operating a regional clinically integrated network built on the concept of a “farmer’s co-operative” to encourage physician participation.
2) Hiring local community care coordinators to work one on one with target beneficiaries.
3) Using patient portal technology and personalized incentives to better engage beneficiaries in their care.
4) Using community care coordination software to connect healthcare providers, emergency medical services, and community social service organizations to help manage patient care.
5) Engaging local community service organizations and emergency medical services to work with health care providers as part of a team based patient centered medical home model.
6) Developing a sustainable pay for value payment program with payers.
The six components of the new care delivery model/network are designed to help address the significant challenges faced by rural health care providers in attempting to implement new care delivery programs and clinically integrated networks.
eTransX is assisting UMTRH in detailed discussions with Medicaid health plans in the region to develop a new pay for value model for this initiative. This value-based healthcare initiative is supportive of a major statewide healthcare payment reform initiative launched by the State of Tennessee that focuses on pay for value programs. In addition, the initiative recently received grant funding through the Medicare Rural Hospital Flexibility Program (referred to as the FLEX Program) to further develop plans for the new value based healthcare model.
