Sense Health Partners with The Greater Buffalo United Accountable Healthcare Network to Enhance Care Management Services for High-Need Medicaid Patients
New York, NY (PRWEB) March 19, 2014 -- Sense Health, an NYC-based healthcare technology company has partnered with the GBUAHN Health Home in Buffalo to create, deliver, and monitor interactive patient support plans, called “Scripts,” for use with Erie County’s Medicaid beneficiaries. Scripts are algorithmically tailored text messages, sent by healthcare professionals to a patient’s phone, that educate, motivate, remind, and check-in on patients and their self-care. Sense Health alerts providers when a patient requires more intensive medical / behavioral attention.
GBUAHN - One of Buffalo’s largest Health Homes – immediately identified the technology’s upsides for Medicaid patients with chronic diseases. “The Health Home is all about integrating medical and mental health services with patient self-care and healthy behavior promotion,” says GBUAHN CEO, Dr. Raul Vazquez. “This platform allows patients to continuously learn how to better manage their health and avoid costly hospitalizations. It’s a win-win.”
“We’re thrilled to be working with the GBUAHN to help them realize their vision of delivering patient centered care that effectively helps people manage their health,” says Brad Hammonds, COO of Sense Health. “We built Sense Health in recognition of the fact that many patient centered care models don’t have nearly enough health providers to deliver the consistent health support to large numbers of patients in need. Our technology was designed to increase the bandwidth of providers in these care delivery models so they can efficiently reach all of their patients.”
The Sense Health platform is one of the few emerging patient engagement technologies that was designed to be able to reach disenfranchised patient populations through its novel use of text messages in care delivery. “Our vision is centered around three driving factors, people need to feel confident and motivated to actually start managing their health, they need to feel connected and accountable to providers in the health continuum, and their support needs to be human and conversation driven, not robotic and instruction based,” says CEO, Stan Berkow. “We know providers don’t have the time to deliver this support to all of the patients who need it, that’s why we built Sense Health, to effectively facilitate such conversations between providers and their patients.”
