Sherman Oaks Hospital Announces Opening of Center for Senior Care
Sherman Oaks, Calif. – (September 2, 2015) -- Sherman Oaks Hospital (SOH) continuously assess the needs of the community it serves. The shortage of health facilities that provide access to care specific to seniors, serves as barriers to living better, longer, and healthier. To respond to this demand, Sherman Oaks Hospital developed a one-of-a-kind Comprehensive Senior Care Program. The new Center for Senior Care (CSC) provides for the physical, functional, psychological, and social needs of seniors; deliver preventive care; and address disease management of individuals 65 years of age and older.
The Center for Senior Care program at Sherman Oaks Hospital is an outpatient service led by a board-certified geriatric nurse practitioner, Dr. Tammi Harris. Dr. Harris has dedicated her practice in geriatrics for more than 12 years. Her clinical experience includes care of seniors in acute care setting (hospitals), outpatient clinic, home health, hospice, skilled nursing and rehabilitation, and assisted living. She has a passion for education and is committed to taking care of vulnerable populations.
The Center for Senior Care model utilizes a collaborative approach, involving appropriate disciplines to provide adequate care. Physicians, medical specialists, therapists, pharmacists, and other disciplines work cooperatively to manage the health needs of seniors. The program works with families by providing them with safety tips; and advisement on dealing and coping with aging adults. Indeed a critical feature of this service concerns the patient’s transition of care to home or other health care facilities. The involvement of CSC in the health continuum cycle extends beyond the outpatient setting. Care coordination is an essential feature of the program and is designed to adequately address priority health issues and prevent exacerbations or deteriorations that may worsen existing conditions and lead to complications.

Sherman Oaks Hospital will open the Center for Senior Care on October 5, 2015. Regular clinic hours are from 9:00 am to 4:00 pm, Monday – Friday. CSC offers comprehensive screenings, examinations, diagnostics, and medical consultations including psychotherapy, neurology, internal medicine, and others. CSC builds relationships among all care providers to remain connected to the patient’s progress. The aim of the program is for seniors to maintain independence and live life optimally.
New patients are being accepted and walk-ins are welcome.
